Job Summary
Specialty Pharmacy is an exciting, dynamic and continually growing area of pharmacy practice at Michigan Medicine. These medications are high-cost medications used to treat patients with specialized diseases requiring complex treatment and coordinated care. As a member of our highly dedicated specialty pharmacy team, you will deliver comprehensive services for patients receiving self-administered specialty medications across the spectrum of access, delivery, and innovative clinical care. In this role, you will apply your knowledge and expertise of specialty disease state management to provide clinical services including patient assessment, medication review and reconciliation, patient education and regular follow up with patients utilizing our pharmacies for their specialty medication needs.

Responsibilities*
This temporary clinical pharmacist position is dedicated to 100% patient care as a clinical pharmacist, generalist within the specialty pharmacy program. This position reports directly to the Assistant Director ? Specialty Clinical Services. This position will work a hybrid work schedule, with primarily remote work with potential for onsite work pending space and future planning. Remote work must be done inside the state of Michigan.
Characteristic Duties and Responsibilities
The pharmacist will support patient care responsibilities for University of Michigan Health specialty pharmacy patients. This includes developing individualized patient care plans and performing therapy baseline and periodic reassessment for every enrolled patient prior to medication dispensation. Both initial and reassessment involves adherence monitoring, medication reconciliation, drug interaction assessment, side effect management, comprehensive medication reviews (CMR), and patient counseling and education. The clinical pharmacist generalist also supports clinic providers in selecting appropriate specialty medication(s), responding to patient-specific drug information requests, and writing insurance coverage appeal letters for specialty medications.
Core functions include:
Patient care duties include prescription verification, clinical care for specialty patients including, but not limited to, baseline and follow-up CMR and adherence checks, supporting clinical services in conjunction with UMHS providers and clinics, and close coordination with UM Health clinics providing care to specialty patients.
Pharmacist responsibilities are focused to ensure optimal effectiveness and safety of medications dispensed and to consistently provide accurate, clear, and timely drug information to members of healthcare teams and patients.
The incumbent will provide functional supervision of technical staff working in their area including completion of job assignments and assuring adherence to institutional policies and procedures. Additionally, the incumbent will contribute to accreditation maintenance and renewal along with associated quality improvement and assurance initiatives and metrics.

Licensure & Certification
Current registration as a Registered Pharmacist by the Michigan State Board of Pharmacy is necessary. Possession of a state narcotics license is necessary. This classification requires Primary Source Verification.
